Family and Child Health Services Homes Visitation Nurse (Per Diem)

Part-time, up to 19 hours per week Position Summary: The Family and Child Health Home Visiting Nurse, under the limited supervision of the Nursing Supervisor and the Public Health Director, is primarily responsible for the provision of home-based visitation services in the CaCoon Program. Duties include long-term nurse case management with children and families. CaCoon is a nurse home visiting program for children and youth with special health care needs (CYSHCN) from birth through age 21 and their caregivers. Services include care coordination to ensure access to needed medical, educational, and social supports. This position works with culturally diverse families in communities with broad based socio-inequity. May also assist with immunization clinics (on/offsite), screen and follow up regarding communicable disease, and provide family planning services and education. Occasionally it requires overnight travel for training and meetings. Work schedule flexibility is necessary. Frequently exposed to air and blood borne pathogens because of continuous contact with the public, children and homes. This position is grant-funded. If funding ceases, this position may be vacated. Supervisory Responsibilities: None Job Duties/Responsibilities:

  • Seeks, enrolls, and delivers home-visiting services to children/families identified to be at risk. Must be able to work effectively with families and individuals of diverse backgrounds (including teen parents; families with mental health, substance abuse, or domestic violence concerns and families experiencing poverty) while establishing trusting relationships and maintaining professional boundaries.
  • Must be able to function autonomously using independent judgment in making decisions, and resolving problems and issues impacting the health and welfare of clients and the general public.
  • Formulates and implements plans for nursing and general health care for individuals and families based on evaluation of needs with a focus on meeting mutually accepted health goals and objectives. Delivers nursing services through clinics, home visits, case management and community outreach programs.
  • Promotes the maintenance of health through teaching and counseling, conducting needs assessment and case management, providing direct client care and/or resource coordination and employing appropriate preventative treatment and health maintenance measures.
  • Encourage and support parent involvement and participation to promote positive parent-child relationships, support healthy growth and development, enhance problem-solving skills and improve support systems.
  • Use a variety of equipment, activities, and opportunities to promote age appropriate development of infants and toddlers in all developmental areas within a home environment structured to promote safety.
  • Connect families with community resources and support self-advocacy across systems.
  • Coordinate and collaborate with child care and other agencies to ensure the child/family is receiving comprehensive early childhood services.
  • Display time management skills needed to maintain accurate, legible, timely documentation of health history, assessments, goals and objectives and care provided in compliance with department and program policy, procedure and protocol.
  • Maintain sensitive and confidential client information according to the HIPAA policy.
  • Fulfill all training and evaluation requirements and maintain an individual training plan as specified on performance assessments.
  • Serve as a cohesive member of the team, participate in problem solving and promote innovation. Collaborates with related community agencies and service providers through client referrals, written and verbal communications and case/team conferences.
  • Meet regularly with the Nursing supervisor and/or Public Health Director for support, guidance and case conferencing.
  • Participate in health fairs, open houses and other community events to promote Public Health programs and services as directed.
  • Employee responds to public health emergencies, incidents and events and participates in all exercises, drills and trainings on emergency preparedness as required. May be required in Public Health Emergencies to work irregular hours.
  • Performs related duties as assigned to meet business needs.
