Field Case Management

Work from home Full-time role Hiring

Duration: 2 contracts Position Summary:

  • The Field Case Management Coordinator is responsible for supporting elderly individuals and people with disabilities who qualify for in-home or nursing facility care.
  • This role utilizes assessment, care planning, coordination, advocacy, and education to help members access appropriate healthcare services and community resources while improving health outcomes and promoting independence.
  • This is a field-based position requiring travel to members' homes within designated counties in Illinois. Candidates must be highly organized, self-motivated, and comfortable working independently while managing member visits and documentation remotely between appointments.

Key Responsibilities: Member Evaluation & Care Coordination

  • Conduct comprehensive evaluations of member needs, eligibility, and healthcare requirements using care management tools and data review.
  • Assess member benefit plans and identify appropriate internal and external resources/services.
  • Identify high-risk factors and service needs impacting care outcomes and initiate referrals when needed.
  • Develop, coordinate, implement, and monitor individualized care plans.

Quality of Care & Member Support

  • Collaborate with case managers, supervisors, medical directors, providers, and community organizations to improve member outcomes.
  • Participate in case reviews and multidisciplinary discussions when necessary.
  • Utilize motivational interviewing and member engagement strategies to encourage healthy lifestyle changes.
  • Provide education, coaching, and support to help members make informed healthcare decisions.
  • Advocate for services and solutions aligned with member healthcare and benefit needs.

Documentation & Compliance

  • Maintain accurate case documentation and ensure compliance with quality management processes, regulatory standards, and company procedures.
  • Monitor member progress and care plan effectiveness.

Required Qualifications

  • Bachelor's degree in Behavioral Health or Human Services REQUIRED, including fields such as:
  • Psychology
  • Social Work
  • Marriage & Family Therapy
  • Counseling
  • Nursing
  • Human Services

(Non-licensed Master's-level clinicians also accepted.)

  • Minimum 2 years of experience in behavioral health, social services, or related field.

Preferred Qualifications

  • Case management experience preferred
  • Managed care experience preferred
  • Experience supporting elderly or disabled populations
  • Strong critical thinking, communication, and organizational skills
  • Ability to work independently and travel locally for member home visits
