Remote Case Manager, Social Services

Our client is seeking a compassionate and dedicated Remote Case Manager to join their Community and Social Care division. Key Responsibilities

Strong understanding of social service systems, community resources, and available support programs. Excellent assessment, interviewing, and active listening skills. Proficiency in developing and implementing effective service plans. Strong advocacy skills with the ability to navigate reputed company systems on behalf of clients. Exceptional organizational and time management skills, with the ability to manage a caseload effectively. Proficient in using case management software and electronic health records (EHR) systems.
