[Remote] Community Liaison - Healthcare Partnerships

They are seeking a Community Liaison to lead outreach and engagement with healthcare partners, helping residents navigate nutrition benefits and build connections that support healthier lives.

Responsibilities

  • Build and maintain strong relationships with community partners, healthcare providers, and local organizations
  • Plan and attend community events, workshops, and outreach activities to promote Mercato’s programs
  • Educate residents about eligibility and enrollment for nutrition benefit programs
  • Coordinate with internal teams to align outreach with statewide health initiatives
  • Track engagement metrics, partnerships, and program outcomes
  • Represent Mercato in the community with professionalism, empathy, and enthusiasm
  • Screen and Navigate residents to service providers to ensure access to eligible services

Skills

  • 1–2 years in community outreach, healthcare navigation, or public service
  • Excellent communication, relationship-building, and organizational skills
  • Culturally competent, adaptable, and mission-driven
  • Comfortable with Microsoft Office or Google Workspace; CRM tools a plus
  • Bachelor's degree in public health, social work, or a related field—or equivalent experience
  • Flexibility to work some evenings or weekends for community events
  • Familiarity with Staten Island neighborhoods and community networks is highly preferred
